Polish Passport Information
A Polish passport establishes your citizenship and allows you to travel abroad. It is printed in Poland, no matter where you are currently living. The document is valid for 10 years for adults and 5 years for children between 5 and 13 years old. However, it can take up to six months for processing, so applicants should plan well in advance. Applicants must appear in person at the Polish Consulate. Applications are not accepted in the mail except in very rare circumstances.
Application Form

Obtain two passport "questionairres" or applications from the nearest Polish Consulate. They cannot be printed online because they must be on special paper.

Instructions

Fill out the applications using block letters, including Polish characters. Make sure both are completed fully and signed.

Photographs

Obtain three recent passport-sized photographs. They must be taken within the last six months, be on a clear background and show your full face with a closed mouth. Glasses and headgear are not permitted. Sign your name, surname and date of birth on the back of one with pencil.

Documents

Submit an original or notorized copy of your birth certificate. If you're applying for a new passport because of a change in name, submit a marriage license.

Lost Passport

Explain the loss of your previous passport if you're applying for a new one for that reason. You must request a form from the Consulate to complete, explaining when and how the passport was lost.

Fees

A 10-year passport costs about $100. There is a 50 percent discount for students and veterans. The fee is $190 to replace a lost passport.
