The Czech Republic is a Central European country, adjacent to Germany, Poland, Austria and Slovakia. While the standard Czech national identity card allows Czech citizens to travel throughout Europe without a visa or passport, individuals must possess a Czech passport to travel outside Europe. Similarly, most immigrants need a visa to reside in the country. Thus, it is important to ensure both documents are accurate. Learn how to change the name on these Czech travel documents to ensure accuracy when your name changes due to marriage, divorce or similar civic formalities.

Step 1

Schedule an appointment with the closest Czech embassy in your state, province or country (see link in Resources). Czech passport and visa processing and modifications must be carried out in-person and may not be submitted via postal mail, fax or electronically through the Internet. When talking to an embassy representative, you will need to provide the name on the passport or visa, as well as your passport or visa number.

Step 2

Obtain civil documents explaining your name change. For example, individuals who have changed their name through marriage or divorce must possess a marriage or court certificate documenting the change. Individuals who changed their name through a formal process with their local government must hold a notarized, dated document proving the official change.

Step 3

Gather additional documents if you are changing your name on a Czech passport. Doing so requires you to apply for a new Czech passport. You will need a current Czech Republic national identity card and a single passport photo. The photo must measure 5-by-5cm and have been taken within the 6 months preceding your passport application. In addition, you must pay the processing fee of CZK $1,200 at the time you submit your name change and new passport application.

Step 4

Bring in your current Czech visa or passport on the day of your appointment, along with the aforementioned documents. If changing a name on a visa, ensure both the visa and the foreign passport in which the visa is inserted are current.

Step 5

Submit documents to the embassy representative with the appropriate form to change your name on your Czech passport or visa. The embassy official will direct you to the correct form, as it will differ in accordance to your passport or visa type. Processing time may take as little as 1 day for visas and up to several weeks for passport changes.
